Three steps are suggested for participants or family members interested in adult day services:

1) Tour

Families should bring the prospective client for a site tour and information. At that time, the prospective participant is evaluated for appropriateness for the program.

2) Physician’s medical evaluation

Clients must be under the care of a physician, with a pre-admission medical form preferred. In emergencies, the service may be provided before the medical form is completed as long as it is submitted within 30 days of admission.

3) Admission form completed

After the first two steps, an appointment should be made with the program director for completion of the state-required forms. At this time, the admission date is determined.
