Obtaining Diakon Help At Home is as easy as making a telephone call. It will take our staff members only a few minutes to determine basic information and set up a time for a professional care manager to visit the customer’s home. Our care manager follows the customer’s care throughout his or her relationship with Diakon Help At Home. The care manager also helps to carefully select a home care assistant meeting each customer’s personal preferences and needs.


During the visit, the professional care manager assesses all the customer’s needs and creates a customized service plan that will help the customer remain independent in his or her own home. There is no cost for this initial intake and the customer is free to accept the entire plan or to pick and choose specific services.
