Diakon Lutheran Social Ministries offers an array of services designed
to help people remain independent and in the comfort of their own
homes.
Diakon Volunteer
Home Care provides short-term, non-medical assistance-such
as companionship, errands, transportation, light household chores,
and meal planning-through teams of volunteers from local communities
and congregations.
If, however, you need more intensive assistance on a respite, short,
or longer-term basis, Diakon Help at Home may be
your answer. This fee-for-service program includes an initial visit
by a professional care manager who assesses needs and creates a customized
plan to help the client remain independent. Services include meal
planning and preparation, bathing , grooming, dressing, home exercises,
medication monitoring, transportation, and more.
Diakon Home Health offers home health
care to individuals who are homebound and facing conditions that are
complex, difficult to manage, and affect quality of life, such as
cancer, heart disease, stroke, and pulmonary disease. Palliative care,
or home health, focuses on comfort care, symptom management, supportive
services, and health care education.
