Affiliate Providers
Diakon Inroads EAP is continuously looking for Affiliate providers to expand and
enhance our network. It is important to us to provide outstanding service to our
customers in a timely, easily accessible manner. Additionally, we understand the
pressure placed on providers. Paperwork demands and the justification needed for
continued service take time and energy away from the reason we got into this
field in the first place…to help people. Our paperwork is minimal and the full
number of sessions are authorized immediately. Diakon Inroads EAP has
streamlined our processes to improve both our clients and your customer
satisfaction.
If you are interested in becoming a provider, please complete our online
information request form. If you want to apply as a group practice, please
fill out the form accordingly. We will reimburse you $60.00 per hour of
counseling with additional fees negotiated for other services. Our reimbursement
process has also been streamlined for easy, efficient payment.
