Diakon Home Health offers skilled nursing and related care, on an intermittent basis, to patients of any age living in portions of eastern and central Pennsylvania.
People receiving care through Diakon Home Health are homebound—that is, they require considerable effort and assistance to leave the home—and need the skills of a registered nurse or therapist as ordered by a physician.
Members of the Diakon Home Health team can help 24 hours a day, seven days a week, on an individual, as-needed basis, as ordered by a physician. Our specialized services include ...

Our team includes your physician, registered nurses, certified home health aides, rehabilitation therapists, and medical social workers. Eligible home health services are covered by Medicare, Medical Assistance, Health Maintenance Organizations, and private insurance plans. Services also can be provided on a fee-for-service basis.
