Three steps are suggested for participants or family members interested in
adult day care:
1) Tour
Families should bring the prospective client for a site tour and
information. At that time, the prospective participant is evaluated for
appropriateness for the program.
2) Physician's Medical Evaluation
In order to adhere to Pennsylvania guidelines, a pre-admission medical
form must be completed. As a part of the process, the prospective
participant must be screened for Tuberculosis by either a Manitou test or
a chest x-ray.
3) Admission forms completion
After the above steps are completed, an appointment is made with the
director for completion of the state-required forms. At this time, the
admission date is determined.
