Diakon Lutheran Social Ministries offers rehabilitation services at its senior living communities in Pennsylvania and Maryland. The programs feature individualized care plans with continuing health assessment and monitoring, therapy offered six to seven days a week; exercise and strengthening programs, on-site medical director, medication administration, and pharmacy services, as well as in-home visits prior to discharge.

Several of our communities also work with local colleges and universities to provide internship sites, where students gain hands-on experience under the watchful eye of our experienced staff.
