Diakon Lutheran Social Ministries has been serving older adults since the 1940s. Today, Diakon offers a continuum of senior living accommodations, including Continuing Care Retirement Communities, home and apartment residences, rental-assistance housing, personal care and assisted living services, memory-enhancement programs, and short- and long-term nursing and rehabilitative care, and related services including adult day care, home care, hospice services, and community-based programs.

• Adult Day Services
  Programs provide daytime assistance that allows older persons or those with disabilities to continue to live in their own homes or with family members, helping to maintain as much independence as possible.
• Community Services for Seniors
  Variety of community-based programs, range from senior centers and meals on wheels to specialized programs for better understanding of health-care systems.

• Diakon Rehabilitation Services
  Individualized rehabilitation services to help you regain the ability to function at your maximum potential.
• Diakon Volunteer Home Care
  Provides short-term, non-medical assistance, such as grocery-shopping and errand assistance and transportation through teams of volunteers from local communities and congregations.
