Three steps are suggested for participants or family members interested in adult day services:

1) Tour

Families and the prospective client are invited for a site tour and to gain additional information.

2) Physician’s medical evaluation

Clients must be under the care of a physician. A signed physician order form must be completed prior to admission (this form is provided by Diakon Adult Day Services at Ravenwood). A PPD (a test for tuberculosis) also is compleeted. We will work with your personal physician to obtain the physician order form and, with your doctor’s approval, can administer the PPD test at the center.

3) Admission form completed

Review of paperwork, including emergency contacts, is done with the center director.
