Diakon Expands Community Impact with New Programs for Families, Youth, and Seniors

Diakon is launching several new programs this year aimed at strengthening families, supporting youth, and improving the well-being of older adults in communities across Pennsylvania. These efforts build on Diakon’s long-standing commitment to providing practical, compassionate support that meets people where they are.
Children’s Review Team – Family Leadership Support (FLS)
Launched in July, the Children’s Review Team in Chester County works to coordinate care across child-serving systems and ensure that youth and families receive comprehensive, collaborative support. By focusing on early intervention and ongoing coordination, the program aims to reduce the need for youth to enter residential treatment and shorten the duration of their placement. Throughout every stage, from intake to discharge and follow-up, Diakon facilitates timely referrals, empowers family voices, and promotes continuity of care to achieve better outcomes.
Senior Peer Support Program – Luzerne County
Also, beginning this summer, this new initiative offers peer-based emotional support to adults aged 60 and older. Volunteer peer supporters will check in with seniors at least once per week to assess overall mental, emotional, and physical well-being. Participants will also receive connections to local resources as needed to help them maintain health and independence in their communities.
In addition to these new initiatives, Diakon has expanded its family reunification and preservation program to Adams County and will soon begin offering its Family Finding program in Berks County, as well as in York County. Diakon Behavioral Health Services is also preparing to launch outpatient counseling services in Chester County in early 2026.
